Please contact Medical Staff Services at (509) 573-3493 for information on credentialing. The pre-applications below include criteria for membership and/or privileges. Completed pre-applications should be returned to Medical Staff Services. Once it is determined that an applicant meets the minimum qualifications, a full application will be provided.

Pre-Applications

Physicians:  Pre-application for Medical Staff (MD, DO, DDS, DMD, DPM, Psychologists only)
ARNPs/PAs:  Pre-application for Allied Health Professionals (PA's and ARNP's only)
